All fees are payable by cash or check at the time of service for treatment sessions and following the completion of an evaluation. Please make checks payable to Kuiper Neurobehavioral Clinic, Inc.
The Clinic is ONLY contracted with Medicare,Tricare Ceridian, and MHN insurance companies. The clinic is NOT a network provider for any other insurance plans. Insurance coverage varies depending on your insurance carrier and your plan. Please note that payment for services is the responsibility of the client whether or not insurance reimbursement is received (this does not apply to Tricare, Ceridian, MHN or Medicare clients).
The Clinic will assist you in filing your insurance claim by providing you with a monthly statement of charges. We will also complete one or two pages of insurance information at no charge. Additional fees will be assessed for forms longer than two pages. All materials for insurance companies will be sent to YOU (except Medicare,Tricare, MHN and Ceridian insurance companies) so that you can be informed before sending them to your insurance carrier. Our policy is to provide communication regarding clinical information directly to you to share with insurance carriers at your discretion.
Insurance Information for Medicare Recipients and Military Patients

The Clinic will file your insurance claim directly. We will provide you with a monthly statement of charges so you can be kept apprised of your claims status.
